Program Description
The FDA receives more than 100,000 reports related to medication errors annually, and an estimated 7000 to 9000 deaths each year are attributed to these errors. Medication errors can occur at any point in the complex medication-use process and continue to occur despite prevention strategies. It is essential for pharmacists in all practice settings to be aware of high-risk medications or practices that increase the risk for errors as well as the importance of error reporting and analysis to identify and correct safety gaps. This activity will empower pharmacists to identify potential areas in which medication errors can occur and develop strategies to better prevent them to ensure patient safety.
