Program Description
Patients with diabetes commonly have comorbid conditions affecting the cardiovascular system and the kidneys. Research indicates that damage to small blood vessels occurring due to diabetes and high blood pressure may be predictors for development of chronic kidney disease (CKD). While medication therapy has led to declines in cardiovascular disease-related mortality, incidence of CKD progressing to end-stage renal disease (ESRD) continues to be a high-risk complication for persons with diabetes. Goals of treatment have been to minimize complications through lifestyle interventions and management of hyperglycemia and hypertension. Pharmacists on the supportive care team have important roles in filling communication gaps between providers, identifying patients with diabetes with comorbid conditions who may be at risk for developing CKD, and helping to manage medication regimens. Investigations into currently available steroidal mineralocorticoid receptor antagonists (MRAs) have demonstrated reduction in cardiovascular mortality and are currently under investigation for use in patients with type 2 diabetes and CKD. This activity will delve into emerging clinical data for MRAs and present interventions pharmacists can make to help improve outcomes of patients with type 2 diabetes and CKD.

Educational Objectives
At the completion of this activity, participants will be able to:
  • Identify the epidemiology of diabetes and chronic kidney disease (CKD) as well as the pathology of kidney disease in this patient population and the impact of inflammation and fibrosis as a progressive factor
  • Analyze current guidelines for CKD and type 2 diabetes (T2D) and the clinical trial data surrounding the emerging nonsteroidal mineralocorticoid receptor antagonists in treatment
  • Illustrate the impact of pharmacists in coordination of care and treatment strategies for CKD in patients with T2D
