Utilizing Targeted Agents for Treatment of HFrEF: The Role of the Pharmacist in Medication Selection and Therapy Management

Program Description

Heart failure with reduced ejection fraction (HFrEF) is characterized by structural and/or functional impairment of the left ventricle, which can result in decreased left ventricular ejection fraction and symptoms leading to functional disability and reduced quality of life for patients. Recently updated guidelines reinforced the importance of patient-centered recommendations, early identification of risk factors, and rapid initiation or escalation of GDMT to prevent events related to worsening HF and improve outcomes. Health-system pharmacists can lead efforts to evaluate and optimize current medication regimens for patients with HFrEF and develop protocols to ensure their safe and continued use while patients are hospitalized and through transitions of care. This program will focus on the expanding armamentarium of HF therapies as well as approaches for pharmacists to incorporate these agents into HF treatment pathways for more successful patient outcomes.

• Explore the clinical burden, diagnostic criteria, role of biomarkers, and disease progression associated with chronic heart failure with reduced ejection fraction (HFrEF)
• Compare safety and efficacy data, administration, and place in updated guidelines for emerging therapies for chronic HFrEF
• Implement strategies for pharmacists to optimize transitions of care, promote selection of safe and effective treatment regimens, and address health care disparities to improve outcomes in patients with HFrEF
