Program Description
Type 2 diabetes (T2D), chronic kidney disease (CKD), and cardiovascular disease (CVD) are among the most prevalent conditions in the United States. Often, patients living with T2D are at increased risk for both CVD and CKD. In recent years, increasing trends in diagnosis of these conditions have resulted in the investigation into medications that can improve treatment and management of all 3 of these conditions. New indications in the sodium-glucose cotransporter-2 (SGLT2) inhibitor class in particular have expanded to improve glucose control, CVD outcomes, and safety in patients with CKD. This program will focus on the role of the pharmacist in optimizing use of SGLT2 inhibitors in the treatment and management of diabetes and CVD according to guideline recommendations and highlight their emerging data in CKD. The included patient counseling video vignettes will present opportunities for pharmacists to improve identification of patients with one or more of these overlapping conditions and engage in shared decision-making throughout treatment, management, and monitoring to improve outcomes with SGLT2 inhibitors.
