Medication errors can occur in all types of health care settings, and when they do occur, they can place patients and staff at great risk of harm. Pharmacists and pharmacy technicians in community pharmacies as well as within health care systems have an opportunity to mitigate patient harm and improve systems to promote patient safety. This article will review the steps at which medication-related problems occur most commonly and present solutions recommended by leading organizations that pharmacists can use to develop risk-reduction strategies within their institutions.

Educational Objectives

At the completion of this activity, pharmacists will be able to:
