The Evolving Treatment Paradigm for the Management of Chronic Worsening Heart Failure: Considerations for Pharmacists

Program Description

The prevalence of heart failure (HF) in the United States is expected to reach 8 million individuals by 2030. Quality of life can be significantly impacted by this complex diagnosis, which is often characterized by frequent encounters with the health care system and recurrent hospitalizations for acute worsening. Although there are effective therapies for management of HF, patients with heart failure with reduced ejection fraction (HFrEF) in particular often have several accompanying comorbidities and are burdened with polypharmacy. Pharmacists in all practice settings can provide valuable care to patients with HF by optimizing transitions of care and providing counseling, education, and medication therapy management services. Pharmacists can help patients and/or caregivers better understand how to manage HF and the complex treatment regimen. This program will provide pharmacists with an update on the current state of HFrEF management with a focus on new therapies and updates in the use of biomarkers for monitoring response to treatment.

Educational Objectives

At the completion of this activity, participants will be able to:

• Review the challenges, clinical burden, associated comorbidities, and the emerging role of biomarkers in heart failure (HF).
• Describe the clinical efficacy and safety data associated with soluble guanylate cyclase (sCG) stimulators and cardiac myosin activators in the treatment of chronic worsening HF.
• Outline the role of the pharmacist in optimizing care of chronic HF by providing patient education, encouraging adherence, and managing adverse effects and comorbidities.
