Program Description
Transitions of care (TOC) is defined as the movement from one setting of care to another. It is estimated that 60% of all medication errors occur during TOC. Novel approaches have been pursued and implemented among various organizations in an effort to improve various stages of TOC. Pharmacists are critical members of the health care team that can prevent and reduce medication errors in order to optimize and advance TOC-related outcomes. This webinar activity explores the medication use-related risks during various phases of the TOC continuum and will compare selected examples of TOC continuous quality improvement projects
