Program Description:
As the treatment landscape for hormone receptor–positive (HR+)/HER2– metastatic breast cancer grows more complex, clinicians still face questions about what to do after a patient progresses on a cyclin-dependent kinase 4/6 inhibitor. Sequencing decisions, molecular testing strategies, and toxicity management vary widely from one institution to the next, often shaped more by practice culture and clinician familiarity than by consistent, evidence-based pathways. Oral selective endocrine receptor degraders (SERDs) are emerging as a critical option in this setting, providing a mechanistically distinct treatment option for appropriately selected patients. However, their safe and effective use depends on interpreting complex biomarker data, navigating payer and testing workflows, and managing a distinct set of toxicities. Oncology and specialty pharmacists sit at the center of these decisions, with responsibilities to interpret ESR1 and comutation results, assess therapy options, and support patients in safe medication use. This activity equips pharmacists to meet these responsibilities through an in-person workshop featuring case-based, small-group problem-solving on sequencing, testing, and toxicity management. Participants will discuss real-world challenges and come away with practical frameworks and ready-to-use tools to apply within their own institutions to anticipate patient barriers and maintain patients on continuous treatment with SERD-based regimens.

Educational Objectives
At the completion of this activity, participants will be able to:
- Examine evidence-based approaches to position, select, and sequence oral selective estrogen receptor degraders (SERDs) and SERD-based combinations within the hormone receptor-positive (HR+)/HER2– metastatic breast cancer treatment continuum, incorporating endocrine sensitivity, prior therapy/tolerability, comorbidities, and patient preferences
- Apply ESR1, co-mutation, and circulating tumor DNA testing principles to determine when testing is appropriate, interpret results in the context of clinical utility, and operationalize pharmacist-led workflows that support timely, feasible, and payer-aligned use of oral SERDs in HR+/HER2– metastatic breast cancer
- Apply monitoring, supportive care, and adherence strategies to prevent, detect, and manage ocular, metabolic, hematologic, and gastrointestinal toxicities of oral SERDs and SERD-based combinations
