Program Description
Cardiorenal-metabolic (CRM) syndrome refers to the overlapping burden of type 2 diabetes, heart failure, and chronic kidney disease, and it is a growing clinical and economic challenge across the US health care system. Shared pathophysiologic pathways drive these conditions to progress more rapidly, contributing to hospitalizations and escalating total cost of care. However, underdiagnosis, clinical inertia, and siloed management continue to leave many patients without the coordinated, proactive care they need. Sodium-glucose cotransporter-2 (SGLT2) inhibitors have reshaped this treatment landscape, with an expanding evidence base demonstrating cardiovascular and renal benefits that extend beyond glycemic control in patients without diabetes. As landmark cardiovascular outcomes, heart failure, and kidney disease trials have accumulated, major guidelines have converged to position SGLT2 inhibitors as foundational therapy for heart failure and chronic kidney disease, independent of hemoglobin A1c. Managed care pharmacists are central to translating this evidence into practice, as they are tasked with designing formulary and utilization management strategies that reflect guideline-directed use. When optimized, these practices can reduce administrative barriers that contribute to clinical inertia and demonstrate value through cost-effectiveness and real-world outcomes data. This session equips managed care professionals and pharmacists to assess the shared mechanisms and economic burden linking conditions in CRM syndrome, evaluate the clinical trial evidence and guideline recommendations that shape SGLT2 inhibitor use, and develop evidence-based formulary and benefit strategies that align real-world evidence with population health goals.

- Examine clinical trial evidence, guideline recommendations, and comparative data on sodium-glucose cotransporter-2 inhibitors in the cardiorenal-metabolic continuum for patient-centered care strategies
- Analyze evidence-based formulary and benefit strategies that incorporate real-world evidence, cost-effectiveness data, and outcomes-based frameworks
