Program Description
The availability of direct oral anticoagulants (DOACs) has improved outcomes for patients with atrial fibrillation or at risk of other thrombotic complications. Concerns remain when using DOACs due to lack of specific antidotes to reverse anticoagulation effects in the setting of emergent bleeding risk or when patients require surgery. Health-system pharmacists must be familiar with current reversal management guidelines to accurately evaluate patients who present with life-threatening bleeding events and identify timing of last dose of DOAC and its rate of elimination should a reversal agent be unavailable. Health-system pharmacists can lead efforts to develop specific protocols for use of DOAC reversal agents within their institutions and can organize training for hospital staff regarding preparation, administration, and dosing of these agents. System-level approaches to reversal of DOAC-related bleeding events can reduce delays in administration of reversal agents, thereby controlling bleeds sooner and improving outcomes for patients. Clinical data for agents under investigation for DOAC reversal, as well as real-world data for FDA-approved reversal agents, will be examined and should be considered when developing protocols and procedures.

At the completion of this activity, participants will be able to:
  • Apply the most recent evidence-based guidelines to the selection of direct oral anticoagulant (DOAC) reversal agents in reversal of major bleeding associated with the use of DOACs.
     
  • Explain the mechanisms of action, safety and efficacy, and adverse effects associated with the use of FDA-approved DOAC reversal agents
     
  • Analyze clinical data and real-world evidence comparing the safety benefits of specific reversal agents over prothrombin complex concentrates for reversing life-threatening DOAC-related bleeding
     
  • Identify the role of the health-system pharmacist in improving patient outcomes with clinical recommendations regarding the proper use of DOAC reversal agents, anticoagulation monitoring protocols, and successful transitions of care of patients after a bleeding event
