Program Description

Hepatorenal syndrome (HRS) is a serious complication of advanced chronic liver disease that is associated with high mortality rates. HRS is classified as either acute kidney injury (HRS AKI) or nonacute kidney injury (HRS NAKI). HRS AKI causes a more rapid decline in renal function and can become life-threatening within days whereas HRS NAKI progresses slowly over weeks to months. Unfortunately, the only cure for HRS is liver transplantation, but immediate treatment to maintain kidney function is imperative to increase chances of survival. Pharmacists can assist in the management and supportive care of patients with HRS AKI by identifying risk factors and causes and notifying prescribers of medications that can precipitate AKI. As medication experts, pharmacists can collaborate with the multidisciplinary team to provide recommendations on selection of appropriate agents for managing HRS AKI, necessary dose adjustments or titrations, and monitor for response to therapy as well as assess any potential safety concerns.
