Medication errors are frequent, costly, and potentially deadly. Medication errors can occur at any stage in the medication use process—from prescribing, transcribing, dispensing, counseling, to administering and monitoring. Health information technology and clinical-decision support have made great strides at improving patient safety, but they also often yield unintended consequences and potentially new errors. Community pharmacists can play a vital role in identifying and mitigating medication errors, leveraging technology, and ensuring their patients have optimal health outcomes.
