Program Description

The role of the specialty pharmacist on the multidisciplinary team in managing homozygous familial hypercholesterolemia (HoFH) is expanding with opportunities for identifying patients, facilitating medication access, improving adherence, and optimizing care to improve quality of life. Within the multidisciplinary team, pharmacists can provide support to patients regarding medication education, injection technique training, and management of medication-related adverse effects as well as prevention of cardiovascular complications. This webinar will feature pharmacist-patient counseling vignettes to further demonstrate communication skills and reinforce concepts surrounding treatment recommendations and education on new and emerging specialty medications for HoFH.

Educational Objectives

At the completion of this activity, participants will be able to:

• Examine the role of the specialty pharmacist in optimizing outcomes in homozygous familial hypercholesterolemia (HoFH) through identifying risk factors, collaborating with the multidisciplinary team, and providing patient education
• Explore adverse effects associated with cholesterol-lowering therapies and pharmacist-led monitoring to help patients mitigate complications and optimize outcomes
• Identify opportunities for pharmacists to counsel patients with HoFH on injection technique for emerging therapies, lifestyle modifications to reduce cholesterol, and the importance of treatment adherence
