This activity was originally presented at the 2025 Community Oncology Conference in Lake Buena Vista, Florida on April 30, 2025.
Program Description
Myelofibrosis is a rare myeloproliferative neoplasm that presents with symptoms including bone marrow fibrosis, splenomegaly, anemia, thrombocytopenia, and inflammation. The JAK/STAT pathway has an established role in myelofibrosis pathogenesis, and research has led to the development of JAK inhibitors (JAKi) which provide significant benefits to patients, including spleen reduction and symptom relief. However, choosing the most appropriate agent is challenging due to evolving evidence and medication-specific adverse effects. Oncology pharmacists within community oncology practices must be prepared to aid in adverse effect management as well as guiding the transition from one JAKi to another. This session will emphasize the role of emerging JAKi therapies within myelofibrosis, highlighting recent updates in treatment guidelines, considerations for individualized treatment decisions, and safe transitions of therapy to reduce ruxolitinib discontinuation syndrome. Expert faculty will also share clinical pearls for improving adherence to oral therapies in myelofibrosis management.

Educational Objectives
At the completion of this activity, participants will be able to:
- Examine the role of JAK inhibitors in frontline and subsequent treatment of myelofibrosis based on individual patient characteristics and disease progression
- Analyze clinical approaches to ensure smooth transitions between JAK inhibitor therapies, minimizing the risk of ruxolitinib discontinuation syndrome in patients with myelofibrosis
- Differentiate the distinct adverse effect profiles and management strategies for JAK inhibitors in myelofibrosis, ensuring optimal patient adherence and improved outcomes
