Program Description
Iron deficiency (ID) is a common comorbidity in patients with heart failure (HF) and may lead to anemia in some patients. ID is linked to increased risk and duration of hospitalization and is associated with an increased risk of mortality for those living with HF. Pharmacists must improve recognition of ID in patients with HF, preemptively identify risk factors, and recommend screening. Advances in the understanding of ID in HF coupled with the development and use of intravenous (IV) iron therapies have led to improvements in patient outcomes compared with oral iron. As central members of the care team for patients with HF, pharmacists should understand the clinical data and evolving guideline recommendations supporting the use of IV iron in patients with ID and HF. This program will specifically examine the role of health-system pharmacists in providing patient education on the risk of ID and anemia in HF, dosing regimens or potential adverse effects with IV iron formulations, and transitions of care.
