Pharmacists can play a major role in the management of heart failure (HF). Patients with HF often have multiple comorbidities and polypharmacy, so the role of the pharmacist in optimizing therapy and monitoring adherence is essential to ensure treatment efficacy and prevent worsening HF exacerbations. This final webinar features counseling videos that will explore communication strategies for pharmacists to effectively share patient education on medication interactions and transitions of care for individuals with HF.

• Illustrate the role of the pharmacist in making appropriate recommendations for managing patients with worsening heart failure with reduced ejection fraction (HFrEF)

• Identify best practices to address barriers and challenges that patients with worsening HFrEF commonly encounter

• Explain the importance of patient education and adherence counseling in the management of worsening HFrEF
