Program Description

Pharmacists can be essential in managing complex medication regimens for patients with anemia in chronic kidney disease (CKD). Adverse drug reactions and drug-drug interactions can have a significant impact on costs and health care outcomes for patients and can be further exacerbated by social determinants of health affecting patients with CKD. This module will focus on biologic, socioeconomic, and demographic differences affecting patients with anemia in CKD that can impact disease progression, health literacy, and access to care. The module will also review prevention strategies pharmacists can implement within their communities and practice settings to mitigate health care disparities to improve provision of care for patients with anemia and CKD.
