Program Description

The burden of anemia associated with chronic kidney disease (CKD) is substantial due to treatment challenges and disease complications affecting ideal management in this patient population. Considerations of when to initiate treatment, choice of medication, and laboratory targets as goals of therapy to minimize adverse effects (AEs) of therapy influence treatment decisions in the multidisciplinary management of anemia in CKD. Research has also shown that racial and ethnic minorities have increased rates of CKD, a higher prevalence of early CKD, and a more rapid decline in renal function. As medication experts, pharmacists should be knowledgeable about the emerging new agents and medication regimens to prevent undertreatment of anemia in CKD across patient populations. This activity will review the pathophysiology of anemia in CKD as well as pharmacist-led strategies to limit AEs, address health care disparities, and ensure appropriate access to care for patients experiencing anemia in CKD. Expert video vignettes featuring an interview with a clinical specialist will accompany this article and offer practical information and advice on the management of patients who are at risk for anemia and diagnosed with CKD, along with patient counseling points.
