Approximately 6 million patients in the United States are prescribed anticoagulants annually, and the number of candidates for anticoagulation is expected to grow as the population ages. With an increasing population on warfarin or DOACs comes a risk of bleeding and situations requiring reversal of anticoagulation depending on the clinical scenario. The American College of Cardiology recommends that a pharmacist be consulted in the event a patient requires an anticoagulant reversal agent, so it is essential to understand the appropriate role of these agents. Pharmacists should have a familiarity with the guidelines, laboratory assessments, and nuances of each of the agents used in the management of warfarin or DOAC-associated bleeds to ensure patients are properly managed.

- Identify the use of INR levels to dose anticoagulants appropriately and the drug-drug interactions among anticoagulants and associated agents
