Program Description
Together with pharmacologic therapy, lifestyle modification and comorbidity management are key in improving cardiovascular outcomes and quality of life. Patients with atherosclerotic vascular disease will benefit from a variety of risk factor reduction strategies, including smoking cessation, blood pressure control, and weight management. Pharmacists can help patients achieve risk reduction through comorbidity management and through medication therapy management. This activity will focus on the pharmacist’s role in working with the health care team to improve quality of care and outcomes in patients with CAD and PAD. Clinical vignettes highlighting the patient encounter will be presented and assessed by the faculty.

Educational Objectives
At the completion of this activity, participants will be able to:
- Outline strategies to effectively discuss coronary artery disease (CAD) and peripheral artery disease (PAD) with patients
- Review important counseling points for patients being treated for CAD/PAD and how they can be actively involved in their own treatment
