Program Description

There are several intricacies to consider for direct oral anticoagulant (DOAC) therapies including indications, dosing, and pharmacokinetic profiles. Pharmacists have a key role in ensuring a patient-centered approach when selecting therapy. Pharmacists can also ensure seamless transitions of care, help patients gain access to therapies, and provide education to patients and other health care professionals. In this Continuing the Conversation program, a panel of experts will discuss the current landscape of DOAC use and the role of pharmacists in driving anticoagulation stewardship across practice settings. This insightful discussion will feature patient cases, providing practical examples that illustrate the key role of pharmacists in optimizing DOAC therapy. Pharmacists will gain insight into best practices, fostering safe medication use and enhancing patient care.

Educational Objectives

At the completion of this activity, participants will be able to:

• Examine the role of the pharmacist in determining use of an appropriate direct oral anticoagulant (DOAC) based on patient-specific factors and guideline recommendations.
• Identify challenges associated with transitioning anticoagulation therapy.
• Apply counseling strategies to improve adherence, monitoring, and prevention of major bleeding in patients receiving anticoagulation therapy.
