Program Description
In hormone receptor–positive/HER2-negative (HR+/HER2−) metastatic breast cancer, progression after first-line endocrine therapy plus cyclin-dependent kinase (CDK) 4/6 inhibition is common. The postprogression landscape often becomes complex, as it depends on the disease mutation status and the patient’s experience with adverse effects. These factors affect treatment decision-making between endocrine-based options, targeted therapies, antibody-drug conjugates, or chemotherapy. Oral selective estrogen receptor degraders (SERDs) have emerged as a distinct class of endocrine therapy for patients with HR+/HER2− metastatic breast cancer, and evidence increasingly supports their role in ESR1-mutated, endocrine-sensitive disease after CDK4/6 progression. However, many health plans lack clear frameworks for the role of oral SERDs, and inconsistent prior authorization criteria drive variability in access. This activity examines the biology behind oral SERDs and equips managed care professionals and pharmacists to translate clinical evidence and real-world data into biomarker-linked coverage criteria, treatment sequencing considerations, and prior authorization standards.
Target audience: Oncology Pharmacists
Type of activity: Application
Release date: November 13, 2026
Expiration date: January 12, 2027
Time to complete activity: 1 hour
Learner level: Foundational, Intermediate, Advanced
Fee: Free
Educational Objectives
At the completion of this activity, pharmacists will be able to:
- Examine the changing treatment options for hormone receptor–positive/HER2-negative (HR+/HER2–) metastatic breast cancer, including endocrine resistance biology and the mechanisms of action of oral selective estrogen receptor degraders (SERDs), to understand their influence on patient experience, health care delivery, and overall disease burden
- Apply clinical trial data, real-world evidence, and guidelines on oral SERDs to treatment choices and sequencing in HR+/HER2− metastatic breast cancer
- Determine managed care strategies that include pathway placement, biomarker-linked coverage criteria, prior authorization considerations, and monitoring expectations to optimize oral SERD use, enhance quality of care, and promote sustainable resource utilization
