Program Description

Diffuse large B-cell lymphoma (DLBCL) is the most common form of non-Hodgkin lymphoma that mainly affects older adults. While first-line treatment with R-CHOP achieves a 50%–60% cure rate, there are still risks of patient mortality, disease that is refractory to initial therapy, and disease relapse within 2 years. Chimeric antigen receptor (CAR) T-cell therapy is effective in treating relapsed/refractory (R/R) DLBCL, however, its potential for relapse and barriers to widespread use highlight the need for alternative therapies. The treatment landscape for R/R DLBCL has since evolved with the introduction of targeted monoclonal antibodies, bispecific antibodies, and antibody-drug conjugates. The optimal treatment sequence for R/R DLBCL therapies remains unclear as these novel agents have overlapping mechanisms of action, therapeutic targets, and approved indications that oncology pharmacists must distinguish within R/R DLBCL based on treatment sequencing recommendations and patient-specific considerations. Expert faculty will provide clinical insights for these therapies that may affect treatment selection for patients with R/R DLBCL, considering second- and third-line treatment options as well as adverse effects, dosing schedule, and supportive care requirements. This session will also provide oncology pharmacists in community-based settings with key dispensing and administration considerations for these agents to improve management of anticancer therapies in R/R DLBCL.

Educational Objectives

At the completion of this activity, participants will be able to:

  • Identify immune-based therapies for relapsed/refractory diffuse large B-cell lymphoma (R/R DLBCL), focusing on their pharmacology, therapeutic positioning, and key dispensing/administration considerations
  • Explore sequencing principles and patient-specific considerations to guide personalized treatment selection for individuals with R/R DLBCL who are not eligible for or have progressed after chimeric antigen receptor T-cell therapy
  • Apply strategies to reduce treatment-related adverse events of immune-based therapies in R/R DLBCL including the use of premedication protocols, dosing adjustments, and patient counseling
