This activity is part 3 of a 3-part curriculum and includes interactive patient counseling video vignettes.

Program Description

Despite the growing body of data supporting continuous glucose monitoring (CGM) devices as the new standard of care for diabetes management, their uptake remains low. Navigating access, understanding data, and integrating CGM into daily routines can be challenging for patients and present opportunities for pharmacist involvement. Community pharmacists are well positioned to identify patients who could benefit from CGM use. In this panel discussion, faculty experts will share strategies to initiate conversations about CGM use with people living with diabetes and tips for helping them access CGM technology. The importance of collaboration between pharmacists, primary care physicians, and other diabetes educators in supporting patients throughout their CGM journey; troubleshooting technical issues; interpreting data; and ensuring successful CGM integration into daily diabetes management will be shared.
