Program Description
Many patients with diabetes do not meet glycemic targets, which can increase risk for complications that increase healthcare resource utilization and decrease patient quality of life. Research has indicated that patients whose diabetes is not adequately controlled are often adversely affected by health disparities and social determinants of health that prevent consistent utilization of healthcare treatment. The use of continuous glucose monitoring (CGM) devices are emerging as new, cost-effective standard of care options to be incorporated into diabetes management plans to enhance patient monitoring and, subsequently, patient outcomes. This supplement will examine real-world evidence and cost-savings models utilizing CGM to assist managed care pharmacists in developing strategies to minimize costs and access barriers associated with CGM for patients with diabetes.
