Program Description
Chronic kidney disease (CKD) is a progressive, life-threatening condition that often coexists with and is significantly worsened by type 2 diabetes (T2D). These interconnected conditions pose significant challenges related to screening, diagnosis, and evidence-based treatment. This webinar will examine the challenges and gaps in CKD management, including issues of under diagnosis and undertreatment as well as the latest treatment guidelines and recent clinical data supporting novel pharmacotherapies. A key focus will be a detailed discussion of sodium-glucose cotransporter 2 inhibitors and glucagon-like peptide-1 receptor agonists, including data from pivotal clinical trials highlighting their cardiorenal benefits beyond glycemic control. The program will also explore the pharmacist's vital role in optimizing cardiorenal-metabolic care, from implementing screening strategies and ensuring medication access to monitoring outcomes and coordinating team-based care.

Objectives
At the completion of this activity, participants will be able to:
- Identify patients at increased risk for chronic kidney disease (CKD) and apply guideline-based recommendations for early screening using urine albumin-to-creatinine ratio and estimated glomerular filtration rate
- Explain the cardiovascular risks associated with early-stage CKD and the need for prompt and appropriate intervention
- Determine the role of sodium-glucose cotransporter 2 inhibitors and glucagon-like peptide-1 receptor agonists in managing CKD and reducing cardiovascular risk
- Apply multidisciplinary, team-based approaches to care for patients with cardiorenal-metabolic comorbidities
