
Optimizing Diabetes Care with CGM

As continuous glucose monitoring (CGM) has evolved and become the new frontier in the management of diabetes, community pharmacists have a unique opportunity to provide valuable education to patients and improve clinical outcomes. This CGM Continuing the Conversation Curriculum will provide a comprehensive overview of the role of the community pharmacist in identifying patients who may benefit from CGM, selection of a device, and assisting in the set-up and interpretation of the ambulatory glucose profile as well as best practices to ensure patients meet glucose targets and achieve therapeutic goals. Pharmacists who complete all 5 activities will receive a certificate of completion that demonstrates their dedication to optimizing use of CGM devices for the management of patients with diabetes.
