Casondra Seibert, PharmD, BCGP

Director of Clinical Pharmacy Services for RxArchitects, Inc.

Casondra Seibert, PharmD, BCGP, is a 2017 graduate of Northeast Ohio Medical University (NEOMED) and holds a board certification in geriatrics. In her role, Dr Seibert is focused on the systems level assessment of pharmacy services in the skilled nursing sector. She prides herself in creation of new services and optimization of existing services to further assist pharmacists in practicing at the top of their license.

Dr Seibert is an Adjunct Professor of Pharmaceutical Sciences at the University of Cincinnati James L. Winkle College of Pharmacy (JWCOP). Within this role, she hosts APPE students, provides geriatric content assessments for the Clinical Applications Course, and instructs in the Geriatrics Elective. Dr Seibert is also the Residency Site Coordinator for UC JWCOP Geriatrics PGY1 Program and is active in research focused on measuring pharmacist impact in the skilled nursing setting.

Dr Seibert is very passionate about geriatric care provision and pharmacy student development. She has a very dynamic attitude and patient care approach. She is a huge proponent of pharmacist development, and it is exciting to see what additional advances her career brings to the industry.

Donald C Moore, PharmD, BCPS, BCOP, DPLA

Pharmacist Clinical Coordinator in Hematology/Oncology at Atrium Health’s Levine Cancer Institute.

“Dr Moore is a very accomplished pharmacist and an individual who is very committed to our profession and to every patient he sees every day. He is a hematology/oncology clinical pharmacy specialist at Levine Cancer Institute and subspecializes in the treatment of hematologic conditions both malignant and benign, such as lymphomas, multiple myeloma, CLL, myeloproliferative neoplasms, ITP, PNH, and thrombotic microangiopathies (TTP, aHUS). He has developed a number of clinical initiatives and programs at our institution such as a pharmacy-driven romiplostim dosing service for patients with immune thrombocytopenia (ITP), a pharmacy-driven rapid rituximab protocol for patients with lymphomas, and a DPYD pharmacogenomic testing program that he collaborates with our pharmacology lab. He is currently the Chair of ACCP’s Hematology/Oncology Practice and Research Network. He recently served as the Hematology/Oncology Roundtable Discussion Leader for the ASHP Midyear Clinical Meeting. He also serves on the ASHP Section Advisory Group for Emerging Sciences.

“He is an accomplished researcher and writer, having published dozens of peer-reviewed manuscripts in journals such as British Journal of Haematology, Clinical Lymphoma Myeloma & Leukemia, Annals of Hematology, Leukemia & Lymphoma, Annals of Pharmacotherapy, and Journal of Oncology Pharmacy Practice. He was awarded the 40 Under 40 in Cancer Award by the National Community Oncology Dispensing Association and Swim Across America in 2020. Aside from all his professional accomplishments, Dr Moore is committed to educating students and residents. He has been a mentor for several of our residents at our institution and is also a mentor for inaugural ASHP's Guided Mentorship Program. He also lectures on lymphomas at our local college of pharmacy (Wingate University College of Pharmacy). Above all, Dr Moore has an extraordinary commitment to the patients he sees every day in clinic. Sarah Ferry, PharmD

Pharmacy Manager at Weis Pharmacy

Sarah Ferry has more than 10 years of community pharmacy experience. She was an intern during a pivotal point in pharmacy, when pharmacists could start immunizing, which came full circle as Sarah is now immunizing her community during a deadly pandemic. In addition, she is a PA pharmacists association active member, membership committee, and continues to keep in touch with members of Congress discussing pharmacy issues that directly affect her community. She is a positive, optimistic advocate for the value our profession provides.

When a fire shut down her pharmacy, as a pharmacy manager, she persevered and continued to work extra hours to help open the pharmacy in a temporary location: challenging but add on the complexities of the COVID pandemic. She is a true pillar of the Hazleton community.

Sarah has been a top performer at every store she has ever worked. She fought so hard to get back to working in her own community that she grew up in enduring the fire and other obstacles and has done everything in her power to ensure that those in her community have the most access to care in the best way she can provide.

She is undeniably an incredible alum to our students, as she returned to mentor them in our pre-pharmacy club at Wilkes to inspire, motivate, and support. There is no one quite like Sarah in pharmacy and we are pleased to offer this recognition of her. – Sarah Elias, nominator for Sarah Ferry




Joanne Morgan, CPht, RPhT

Certified Pharmacy Technician

Material and Purchasing Technician at Capital Health Regional Medical Center (CHRMC)


Joanne joined Capital Health System 20 years ago as an overnight technician when she moved to New Jersey. She brought with her a wealth of technician knowledge from her previous North Carolina hospital and offered improvement in the overall workflow in the department. After a few years, she stepped into the day shift position and continued to perform at a high level where other technicians and college APPE students learn from her. In 2014, her excellent performance led to her promotion to the current Pharmacy Purchasing Technician.

Joanne is a hardworking and dedicated technician who possesses a wealth of knowledge in drug purchasing and applies it in her current role. Besides drugs and equipment ordering, I was most impressed with her continually tracking the drug availability trend and pricing increase/decrease, thus leading to significant savings for the department and the hospital. As this is a 340B hospital, she works closely with her 340B coordinator to ensure drug purchases are in full compliance with the current 340B laws. She reviewed drug ordering by evening or weekend staff to avoid incorrect non-contracted drug purchases. Often times, she even made purchases remotely in the evening from home for the staff from CHRMC and also the sister hospital at Hopewell campus. When the department activities were overwhelming or short-staffed, she volunteered to assist the technicians to make drug delivery to patient care units. Her job performance did not go unnoticed and she was recognized by the hospital as one of the employees of the year. Externally, she maintains great relationships with the pharmaceutical sales representatives and Group Purchasing Organization key leaders and even educated them about 340B regulations and pricing. In fact, she was requested to join and speak at the company annual discussion forum. I understand that she has been approached by companies to join them but I am pleased she continues to remain as a Capital Health employee.

In summary, I am very fortunate to work with a talented technician such as Joanne. As a Director of Pharmacy for the past 40 years, I honestly believe she is a truly outstanding technician who well deserves to be recognized for her contribution to the pharmacy profession. − Chester Lau, nominator for Joanne Morgan
