Program Overview:
This unique meeting will feature activities highlighting "real-world" best practices in population healthcare implementation and delivery. Clinical data and operational lessons will be featured throughout the meeting, with a focus on educating healthcare providers on methods to improve delivery of care and outcomes. This meeting is appropriate for payers, providers, accountable care organizations, integrated delivery networks, national quality organizations, and specialty and retail pharmacy healthcare professionals interested in refining their managed care strategies. Topics include COPD, diabetes, mental health, heart failure, and pain management.
